ACDKL Testing Certificate Request Forms
ATTENTION:
To fill out the ONLINE forms, just select the button for your club, then fill in the colored fields on the first sheet titled, “Info Sheet.” The amounts will be totaled for you.
If you need to order EXTRA SUPPLIES, fill in the colored fields only on the sheet titled “Cost Sheet.” The amounts will be totaled for you on both the cost sheet and the form.
Once done, go to Google Sheets Menu Bar and select the “File" pull down menu, then select “Download,” and choose the PDF.
Under the “Export” pull down menu at the top, you will want to select the “Workbook”.
Page Orientation should be “Portrait.”
Under the “Scale” pull down menu, select “Fit to Height.”
Then press the “Export” button at the top.
Once the PDF has been downloaded, email the PDF to Master Nagel at ackdroster@gmail.com.
